Title: Request for Proposal (RFP) in the Prequalification for Renovation/Rehabilitation of Primary Health Facilities (PHCs)

• The purpose of this Request for Proposal (RFP) is to invite innovative and competitive companies that will submit relevant documents to the Kebbi State Primary Healthcare Development Agency (KBSPHCA) for onward pre-qualification for the above works.

• Evidence of Incorporation or registration of company.
• Evidence of registration with Ministerial Tenders Board.
• Tax clearance certificate for the last three (3) years. iv) Evidence of Financial capacity and Bank support.
• List of key professional staff including registration with relevant professional bodies and experience.
• Technical capacity Equipment and Plants.
• Evidence of relevant experiences and good performances on similar projects.
• VAT remittances and registration certificate.
• Evidence of registration with COREN including regulatory bodies.
• Company Financial Statement/Audited Account.
