Preamble
The Federal Medical Centre, Ebute-Metta, Lagos in implementing her year 2022 Capital Projects hereby invites all interested, qualified and reputable Contractors/Suppliers with relevant experience to submit Technical and Financial Bids Two (2) hard copies each for the following projects.

Scope of Works

• Lot 2022 A - Rehabilitation and Equipping of Dental Centre
• Lot 2022 B - Purchase of Endoscopic Equipment
• Lot 2022 C - Purchase of Laboratory Equipment and Reagents
• Lot 2022 D - Construction of Hospital Complex (Clinical Building) using Dry Construction Technology
• Lot 2022 E - Construction of Clinical Block using Wet Construction
• Lot 2022 F - Procurement of Surgical and Ward Equipment
• Lot 2022 G - Procurement of Computers and Accessories
• Lot 2022 H - Procurement of Hospital information Systems (Expansion of Existing System)
• Lot 2022 I - Procurement of Central Patient Monitor System Patient Monitors
• Lot 2022 J- Procurement of Auxiliary Medical Infrastructure
• Lot 2022 K - Renovation of Electrical Power Systems
• Lot 2022 L - Medical Mission in Agege Federal Constituency of Lagos State
• Lot 2022 M - Provision of Fire Truck and Fire Port
• Lot 2022 N -Construction (Revamping) and Equipping of Two (2) Primary Health Care Centres in Two (2) Locations in the Lagos East Senatorial District of Lagos State
• Lot 2022 O - Supply of Ambulances
• Lot 2022 P - Construction of Oncology Building
• Lot 2022 Q - Construction Fittings/Medical Facilities and Equipment for Oncology Building
• Lot 2022 R - Provision of Medical Grant for selected Vulnerable in Surulere
• Lot 2022 S - Supply of Basic Drugs to Primary Health Centers within Lagos East Senatorial District, Lagos State
• Lot 2022 T - Renovation and Equipping of Primary Health Centre and Supply of Drugs in Labora, Lekki
• Lot 2022 U - Provision, Installation and Commissioning of Passenger Lit for the Hospital Building Complex.

Eligibility Requirements
Interested Contractors/Companies are required to submit the following documents in sequential order as listed below:

• Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) Including Form CAC 1.1 or CAC2 and CAC7;
• Evidence of Company Income's Tax Clearance Certificate for the last three (3) years valid till 31" December, 2022;
• Evidence of Pension Clearance Certificate valid till 31" December, 2022 (this requirement is only applicable to bidders whose number of staff is 15 and above)
• Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31" December, 2022 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder's annual turnover is N50m and above)
• Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31" December, 2022;
• Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers submission of Interim Registration Report (IRR) expiring on 31" December, 2022 or valid Certificate issued by BPP
• Swom Affidavit:
  • Disclosing whether or not any officer of the relevant committees of the Federal Medical Centre Ebute-Metta, Lagos or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars
  • That no Director has been convicted in any Country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter,
  • That the Company is not in receivership, the subject of any form of Insolvency or bankruptcy proceedings or the subject of any form of winding up petition or proceedings:
  • That the company does not have existing loan or financial liability with a bank, other financial institution or third party that in classified as doubt, non-performance, bad or whose repayment portion has been outstanding for the last four (4) months
• Company's Audited Accounts for the last three (3) years-2019, 2020 & 2021, duly acknowledged by the Federal Inland Revenue Service (FIRS);
• Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
• Company's Profile with the Curriculum Vitee of Key Staff to be deployed for the project, including coples of their Academic/Professional qualifications such as CORÉN, QSRBN, ARCON, CORBON etc.:
• Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects;
• Works: List of Plants/Equipment with proof of Ownership/Lease Agreement
• Lot L: Evidence of Firm's current Licence issued by the Medical and Dental Council of Nigeria
• Lot S: Evidence of Firm's current Licence issued by the Pharmacists Council of Nigeria;
• For Lot 2022 (M & O), only companies included in the National Automotive Design and Development Council's approved list of local Manufacturers of vehicles or their authorized representatives should bid for the procurement
• Medical & Health Equipment: Letter of Authorisation from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria;
• For Lot 2022 G, evidence of current registration/accreditation with Computer Professionals Registration Council of Nigeria (CPN) and National Information Technology Development Agency (NITDA);
• Electricity Jobs: Evidence of current License issued by Nigerian Electricity Management Services Agency (NEMSA)
• All documents for submission must be transmitted with a Covering/Forwarding letter under the Company Fem's Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No, and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page. duly signed by the authorised officer of the firm.

Application Closing Date
12:00 Noon Tuesday, 14th June, 2022.

Opening of Technical Bids
Only the Technical bids will be opened immediately after the deadline for submission at 12:00 noon Tuesday, 14th June, 2022, in the Seminar Room, Federal Medical Centre, Ebute-Metta, Lagos. All Tenderers, Civil Society Organisation and interested stakeholders are invited to the bid opening:

Collection of Tender Documents
Interested Companies are to collect Bidding Documents at the Procurement Department of the Hospital on evidence of payment of a non-refundable tender fee of N10,000, paid into the Federal Medical Centre, Ebute-Metta, Lagos' Remita Account in any reputable Commercial Bank in Nigeria.

Submission of Tender Documents
Prospective Bidders are to submit two (2) hard copies each of Technical and Financial Bids which are to be Separately & Individually Sealed with wax envelopes and clearly marked "Original" & "Copy" and thereafter put together in a bigger sealed envelope indicating lot/project number, title, name and address of the Bidder at the top left corner of the envelope with bidders GSM (reachable) Number(s) and email Address(s). Please, ensure that you sign the Bid Submission Register and also dropped in the Tender Box located in the office of the Head Of Procurement as the Federal Medical Centre, Ebute-Metta will not be held liable for misplaced or wrongly submitted bids.

General Information

• Bids must be in English language and signed by an official authorized by the bidder.
• Late submission will be returned un-opened;
• All costs will be borne by bidders;
• Attendees at the bids opening shall be required to wash hands, use hand sanitizers, wear mask and observe physical distancing. The Bids Opening will be covered by video recording and invitation link will be sent to bidder who chooses to join the bid opening online;
• Only pre-qualified bidders at technical evaluation will be invited at a later date for financial bid opening, while the financial bids of unsuccessful bidders will be returned un-opened:
• This advertisement for invitation shall neither be constructed as a commitment on the part of Federal Medical Centre, Ebute-Metta, Lagos nor shall it entitle any contractor to make any claim whatsoever and/or seek any indemnity from FMC, EB. Logos;
• The Federal Medical Centre, Ebute-Metta,Lagos is not bound to pre-qualify any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement Act 2007;
