Preamble

Jhpiego Corporation is seeking for reputable vendors for the supply of the various categories of Kits listed below, to be supplied and delivered to its Abuja and state offices. Vendors are encouraged to read the entire document to ensure that they have a clear understanding of jhpiego requirements before submitting bid.

Scope of Work

Item Description

GBV Survivors, Adolescent Girls and PMTCT Clients Kits for health facilities and pregnant women.

Total Quantities of Kits Required:

600 sets of Dignity Kit
103 sets of Delivery kiit
500 sets of Mama Kit
600 sets of Menstrual hygiene kit

Vendors are solely responsible for ensuring that each kit contain the exact quantity of items, well stocked and packed in durable delivery bags.

Successful vendors will provide sample of bags for vetting by ihpiego, while Jhpiego will provide sample of branding material (e.g. logo) to be put conspicuously on each bag.

Note: All LOTS must be quoted separately and not combined as a single quote.

Eligibility Requirements

All applicants should provide the following:

Certificate of registration/Incorporation, memorandum and articles of association and valid Tax Identification Number (TIN).
Vendor must be a licensed/authorized vendor for services to be rendered (please provide supporting documentation).
Company profile (briefly reflecting the core competencies, key management personnel and clearly stating the physical verifiable office address, mobile number and e-mail address).
Proof or license to offer required services as given by the appropriate regulatory body (if applicable).
Minimum of three (3) reputable references active within the last three years form client/organization Note that applicable government taxes will be deducted prior to making payments for items supplied.

Criteria for Vendor Selection

Jhpiego require the services of experience vendors that have provided these items previously to other implementing agencies within Nigeria. Please attach any valid evidence to support such claim (e.g reference letters)

Application Closing Date

4:00pm WAT, 24th August, 2021.

Submission of Documents

All bids should be addressed and delivered to:

Ensure that you put all quotes into bid box at the reception. Also, ensure that you complete the bid work sheet at the reception.

Quotation Should Include the Following for Both Categories of Equipment

The quotation should be enclosed In plain sealed envelope, with the item being quoted for written boldly on the right-hand side of the envelop.
Proof or license to offer required services as given by the responsible regulatory body (if applicable).
Lead-time for Supply
Quote validity of at least 60 days.
Payment terms if different (preferably Net 30 days).

Contract Duration

At the successful delivery of all items as on the approved Purchase Order.
