Invitation to Tender for 2014 Capital Projects at Federal Medical Centre, Gusau

Pursuant to the implementation of its 2014 Capital Budget, the Federal Medical Centre, Gusau hereby invites interested qualified Contractors and Suppliers to bid for the lnstitutions Capital Projects in Construction Work, Supply of Medical Equipment, Drugs and Motor Vehicles as proposed for 2014 Financial Year. The Projects for Prequalification are as follows:

Scope of Work
Package No.1: ConstructIon Work
Lot 1: Construction of Drainages. Walkways, Parking and Landscaping Phase II
Lot 2: Construction of Boreholes, Water Tanks and Reticulation
Note: Prospective Contractors for the above projects apart from satisfying the prequalification requirements below, should at least have 5 years experience and a record of not less than 2no.similar jobs successfully executed in the last 3 years.

Package No.2: Supply of Medical Equipment and Drugs
Lot 3: Furnishing and Equipping of Amenity Ward.
Note: Prospective Suppliers for Lot 3 should in addition to fulfillment of the prequalificaton requirements below, must possess letter of authorization from Federal Government Approved Medical Equipment Manufacturers.

Package No.3: Supply of Motor Vehicles
Lot 4: Purchase of 4no. Ambulances
Lot 5: Purchase of 2no.Toyota Hilux, Camry

Prequalification Requirement
Interested Contractors wishing to carry out the job are required to submit the following documents:

1. Company Profile with curriculum vitae of key Officers including photocopies of relevant professionals/technical qualifications
2. List of verifiable construction equipment indicating ownership or lease agreement (for Construction Work)
3. Evidence of successful completion of similar work with copies of letters of Award and Certificate of Completion
4. Evidence of VAT Registration with TIN No. and past remittance for the last 3 years.
5. Sealed Company Audited Account Report for the last three (3) years (2011 -2013)
6. Bank Reference from a reputable Bank addressed to the Medical Director indicating readiness to make available to Client credit facilities to execute the Project.
7. Evidence of Registration with Corporate Affairs Commission (CAC),
8. Evidence of current Tax Clearance Certificate for the last three (3) years (2011- 2013).
9. Evidence of compliance with PENCOM in accordance with Pension Reform Act 2004 (as amended).
10. Evidence of compliance with amended Industrial Training Fund 2011 Act.
11. A swom affidavit confirming that all information presented are true and correct of the Company and that no Officer of the Centre is a former or present Director, Shareholder or has any pecuniary interest in the Bidder.

• NB: The above listed requirements shall form part of the bid evaluation criteria and non compliance with criteria (7 - 11) will result in absolute rejection of a Bidders Financial Bid.
• Orderly presentations and arrangement scores marks.

Collection of Tender Document
Interested Contractors/Suppliers should collect Tender Document from the Procurement Unit,
Administrative Block, Federal Medical Centre, Gusau upon the payment of non-refundable tender fee (into Contract/Tender registration A/c of the Centre) as follows:

Package No. 1 N30, 000.00
Paclage No.2 N20, 000.00
Package No.3 N20.000.00

Submission of Bids
The Technical and Financial proposals must be prepared as separate documents. Two copies of Technical and two of the financial proposal including the original must be separately packaged in sealed envelope, clearly marked “Technical or Financial Proposal” as appropriate and put together in a single (Bigger) envelope which should be marked as either Tender for:
Construction Work, Supply of Medical Equipment or Supply of Motor Vehicle and addressed to:
